Experience foundational life planning with our 60-minute session, where you will complete a comprehensive intake questionnaire and receive assistance in preparing essential documents such as Advance Directives, Living Wills, and Power of Attorney forms. This service culminates in a custom Life Planning Blueprint portfolio, complete with all documents saved on a USB drive for your convenience and security.

Deluxe Life Planning Service:

Elevate your end-of-life preparations with our Deluxe service, which includes two detailed 60-minute sessions. In addition to all the offerings of the Basic service, this package provides a deep dive into your final wishes, covering detailed arrangements for your memorial, funeral, or cremation. Everything is meticulously organized into a custom Life Planning Blueprint portfolio and provided on a USB drive.

 

Life Planning - Family Session:

Facilitate important family discussions regarding your end-of-life plans with our 60-minute Family Session. A professional mediator aids in conveying your intentions and addressing any family concerns, ensuring that your decisions are understood and respected. This session is crucial for maintaining family harmony and making sure everyone is aligned with your final wishes.

ADVANCE DIRECTIVE:


An advance directive is a legal document that allows you to outline your preferences for medical care if you become unable to communicate or make decisions due to illness or incapacity. It typically includes instructions on the types of medical treatment you wish to receive or avoid and may appoint a healthcare proxy to make decisions on your behalf, ensuring your healthcare aligns with your wishes when you're not able to voice them yourself.

POWER OF ATTORNEY:

A Power of Attorney (POA) is a legal document that grants someone else the authority to act on your behalf in legal or financial matters while you are still capable. The extent of this authority can be specific to certain tasks or broad, but it traditionally does not extend to situations where you become mentally incapacitated unless specified as durable.

LIVING WILL:

LA legal document that specifies the medical treatments you wish to receive or refuse, particularly concerning life-sustaining procedures, if you are unable to communicate your decisions due to illness or incapacity. DURABLE POWER OF ATTORNEY:

 

A Durable Power of Attorney (DPOA) is a legal document that designates a trusted individual to handle your affairs if you become incapacitated. Unlike a standard power of attorney, its authority persists even if you lose the ability to make decisions, covering legal, financial, and healthcare matters seamlessly.
